Unique Options+ Ltd provides a nurse-led specialist complex service that delivers support to adults of all ages. Our service users have either a learning disability, physical disability, duel diagnosis, autism, brain injury and or any combination that may require a more complex care provision. We can care for people who have a PEG, require catheter care, provide support with medication and or any other medical needs.

Our company can also provide additional support for individuals where medical needs or behaviour can be difficult or challenging for a lone carer. We can provide support for anyone with low, medium or high needs with 1-1, 2-1 or 3-1 care. 

Our staff members are trained to manage and monitor behaviour that challenges. We specialise in using the Positive Behaviour Model and the ABC chart. We then use this data to work with, support and encourage the individual to make positive changes going forward. 

We work with the individual, their family members, and the multi-disciplinary team to create an in-depth person-centred assessment, risk assessments and support plans. We complete these as we understand that everyone is unique as an individual, and everybody has differing needs. Having this information is essential as it gives us a better insight and ensures we offer the best care.  Assessments are reviewed by our onsite social worker and the management team regularly.

We support people in supported living, residential and in community/outreach settings. We work with individuals so they can be as independent as possible at home and in the community. Our hours range from 5 hours to 24 hours per day. 

We have a qualified nurse on site who will train support workers to manage the packages which have more complex health requirements, and each worker will only be signed off when competent in the delivery of care. 

Every support worker we employ will be DBS checked and fully compliant with all mandatory training. Our support workers are trained to support any tailored requirements. These training elements include the varying disabilities and how to care for people who are living with them, dual diagnosis, autism, medication, and side effects to look out for, PEG training, catheter care, positive behaviour model, 6C’s of care, and the person-centred value model. 

We ensure each support worker knows how a person would like to receive care before any care/support package begins. We achieve success by creating a package which is tailored to the individual covering their condition, personal care, medication, and support needs. 

Unique Options+ has a vision to give everybody we care for a voice, a choice, and the opportunity to feel a valuable part of society. As well as working in partnership with the individual, their family members, and the multi-disciplinary teams, we also ensure each person has regular health checks and has a hospital passport. We believe this is important as it gives each person the continuity of care from all services.
